Introducing Live Healthy Lane

Live Healthy Lane is a community-based effort to improve the health and well-being of those who live, learn, work, and play in Lane County. This collaborative project includes a formal partnership of key stakeholders working alongside the community to create the conditions for optimal health and well-being.

Together, Lane Community Health CouncilLane County Public HealthPeaceHealth Oregon NetworkTrillium Community Health Plan, and United Way of Lane County oversee the periodic assessment of health issues and needs in Lane County (The Community Health Assessment; CHA) and the development and implementation of the Community Health Improvement Plan (CHP) in collaboration with the community. Reducing health disparities, promoting health equity and improving overall population health are the goals to improve community health for the current CHP.

Community Health Assessment & Community Health Improvement Plan

Lane County’s Community Health Assessment (CHA) and Community Health Improvement Plan (CHP) reports and additional resources are available for the community to use to inform local health efforts.

2018-19 Community Health Assessment

​The 2021-25 Community Health Improvement Plan (CHP) is informed by the 2018-19 CHA, which identified health issues and needs in Lane County.

2018-19 CHA Themes:

  • A significant proportion of Lane County residents lack access to affordable housing, living wage jobs, health food, and quality childcare, all of which are key social determinants of health.
  • There are significant disparities in both health outcomes and the social determinants of health in Lane County based on race/ethnicity.
  • There is a concerning trend of worsening mental health among Lane County residents, especially youth.
  • The above conditions stem from and contribute to social and environmental conditions that are determinants to health behaviors and healthy outcomes for Lane County residents.

2021-25 Community Health Improvement Plan

Lane County’s CHP is a menu of action items that, taken together as a community, have the potential to achieve Live Healthy Lane’s vision of: Working together to create a caring community where all people can live a healthier life.

2021-25 CHP Priorities:

  • Ensure incomes are sufficient to meet basic costs of living (i.e., housing, childcare, food transportation, etc.)
  • Establish community conditions that support behavioral health and physical well-being
  • Address current historical injustices that produce disparities

Coordinated Care Organization Health Promotion Programs

  • Lane County Public Health works with Lane County’s Coordinated Care Organizations (CCOs), Trillium Community Health Plan and PacificSource Community Solutions, to provide upstream prevention programs in Lane County that align with the CHP.
