Community Health Needs Assessment (CHNA)

The 2015 Community Health Needs Assessment is a collaborative project that analyzes data that addresses quality of life, socioeconomic factors, systems and policy level infrastructure, as well as traditional physical and mental health indicators. The overall goal of this work is to identify key priority areas where the community can take action to improve overall population health.

Community Health Improvement Plan (CHIP)

A community health improvement plan (CHIP) sets out to tackle public health problems based on results of community health assessment activities. We use our CHIP as we connect with community partners to help address five key priority areas in Lane County: Access to services; health equity; obesity; substance abuse & addictions; and tobacco.
